Duke Managed Care

Duke employees and their families may be covered by Duke Select, Duke Options, and Duke Classic health plans. These plans require patients receiving infertility services to pay a co-payment based on a packaged group of services. Each plan has a lifetime maximum of three attempts.

  • Package One: In vitro fertilization (IVF). Patient pays $3,500 each cycle.
  • Package Two: IVF with intracytoplasmic sperm injection (ICSI). Patient pays $4,000 each cycle.
  • Package Three: A surgical procedure such as myomectomy or the treatment of endometriosis. Patient pays $3,000.
  • Package Four: Controlled ovarian hyperstimulation (COH). Patient pays $650 each cycle.
  • Package Five: Frozen embryo transfer (FET). Patient pays $600 each cycle.

The drugs that are approved for use in the fertility packages are Repronex®, Follistim®, Bravelle®, Gonal-F®, and human chorionic gonadotropin (hCG). Patients are responsible for 50 percent of the cost of the drugs in addition to the package price.

Note: Patients should use the Merck-Medco PAID Prescriptions card at participating pharmacies to purchase infertility drugs.
